How to switch to the NEW FreeStyle Libre 2 Plus sensor

The FreeStyle Libre 2 Plus sensor is the latest innovation to the FreeStyle Libre 2 system, and is now listed through the NDSS. Whether you're upgrading from FreeStyle Libre 2 sensors, or want to move over from another sensor, follow these steps to switch to FreeStyle Libre 2 Plus sensors.

  1.  Complete the following switch form Continuous and Flash Glucose Monitoring Acces Form: Updating or Ceasing Access
  2. Have your authorised diabetes healthcare professional sign the form.
  3. Your healthcare professional can then lodge the form through the NDSS online portal, or it can be lodged by emailing it to info@ndss.com.au, or post it to GPO Box 9824 in your capital city.

Once the form has been processed, you will receive a notification from the NDSS. Further information is available at ndss.com.au or by calling the NDSS Helpline on 1800 637 700.

aged under 21
