FreeStyle Libre Subscription Start Pack Offer

1. This promotion is being conducted by Abbott Australasia Pty Ltd (ABN 95 000 180 389) of 299 Lane Cove Road, Macquarie Park NSW 2113 (Abbott). This promotion commences on 27 April 2021 at 12:01am and ends on 30 September 2022 at 11:59pm (Promotion Period). By participating in this promotion, the customer agrees to be bound by these Terms and Conditions. 
2. This promotion comprises the following offer (Offer). When a new subscription is placed by a customer for Freestyle Libre 2 sensors for the Subscription Starter Pack offer during the Promotion Period on Abbott’s FreeStyle Libre website at www.scantocheck.com.au (Subscription), the customer will receive the first 28-days’s supply of FreeStyle Libre 2 sensors (two FreeStyle Libre 2 sensors) free. The Offer is subject to the following:

This Offer is only available to Australian residents aged 18 years or older who have not previously made a purchase for FreeStyle Libre or FreeStyle Libre 2 sensors on www.freestylelibre.com.au, and who have not previously placed a subscription for Freestyle Libre or FreeStyle Libre 2 sensors at the time of placing a subscription on Abbott’s FreeStyle Libre website. 
4. The Offer is only available online and only one Offer is available per customer. This Offer is not available in conjunction with any other Abbott promotion or offer. 
5. The Customer will receive the two free FreeStyle Libre 2 sensors (up to 28-days wear time) within the first month of the subscription (the total retail price of two FreeStyle Libre 2 sensors is $185). Should the Customer remain subscribed thereafter, the cost of each28-days supply of two FreeStyle Libre 2 sensors is $185 and will be billed to the Customer. 
6. The Offer, and the FreeStyle Libre 2 sensors provided pursuant to an Offer, must not be re-sold, provided to any third party for reimbursement, transferred, exchanged or redeemed for cash, credit or other products.  
7. Abbott may, in its sole discretion, modify these terms and conditions, the Offer, or the contents of any Offer at any time and any such modification will take effect when posted to this website. 
8. These terms and conditions do not limit or exclude any statutory provisions or statutory consumer rights under applicable law (including those under the Competition and Consumer Act 2010 (Cth)). 
9. Abbott does not guarantee that access to this website, the promotion or Offer will be uninterrupted or free from viruses, technical issues or malfunctions.  
10. If for any reason Abbott is not able to conduct with this promotion (or any part of it) as planned due to any circumstance beyond Abbott’s reasonable control, Abbott reserves the right, in its sole discretion, to cancel, terminate, suspend or postpone the promotion (or any part of it). 
11. To the maximum extent permitted by law, Abbott excludes all liability in respect of any loss, expense, damage, or injury suffered by any person arising out of or in connection with this promotion, including (but not limited to) accessing the promotion or any Offer. 
12. By participating in this promotion, customer agrees that Abbott its affiliates and service providers may use customer’s personal information to conduct this promotion and its business and to contact customer regarding his/her account on the website and the order placed. Personal information is collected, used, stored and handled by Abbott in accordance with its Privacy Policy available at http://www.freestylelibre.com.au/privacy_policy.
