Everything you need to know about upgrading to the new FreeStyle Libre 2 Plus sensor

Let's get started

Select your current purchase or access point

  1.  Complete the following switch form Continuous and Flash Glucose Monitoring Acces Form: Updating or Ceasing Access.
  2. Have your authorised diabetes healthcare professional sign the form.
  3. Your healthcare professional can then lodge the form through the NDSS online portal, or it can be lodged by emailing it to info@ndss.com.au, or post it to GPO Box 9824 in your capital city.
  4. If you require a FreeStyle Libre 2 reader, eligible NDSS users can request a complimentary Freestyle Libre 2 reader here

Once the form has been processed, you will receive a notification from the NDSS. Further information is available at ndss.com.au or by calling the NDSS Helpline on 1800 637 700.

By clicking the button below, you can immediately upgrade your existing subscription(s)to a FreeStyle Libre 2 Plus "Subscribe and Save" subscription. There is no change to the price or delivery dates of your subscription, and you will retain your progress to your next 6th Free Order. If you have multiple subscriptions, all subscriptions will be upgraded. If your subscription is currently paused, it will not automatically resume.

Please note, all subscriptions will automatically be upgraded to FreeStyle Libre 2 Plus subscriptions on 14th April, 2025.

€ In order to use FreeStyle Libre 2 sensor, you need to ensure you have a compatible smartphone and the latest FreeStyle LibreLink app downloaded. If you’re using the FreeStyle Libre sensor with the FreeStyle LibreLink app, you may go on to switch to the FreeStyle Libre 2 sensor, but if you’re using your FreeStyle Libre sensor with the FreeStyle Libre reader you will have to switch to the FreeStyle LibreLink app. The FreeStyle Libre 2 sensor will only work with the FreeStyle LibreLink app. You may continue to use the FreeStyle Libre sensor with the FreeStyle Libre reader if you do not have a compatible smartphone.
